If you have previously been assessed by Bedford Borough Council's Adults' Services as a self-funder, you can now complete an online care plan review to help us keep a record of your care needs and preferences.

Who is a self-funder?

You are considered a self-funder if you pay for and manage your own care and support services without financial assistance or direct involvement from the local authority. 

This may be because:

  • you have chosen to arrange your own care independently
  • your financial situation means you are not eligible for local authority funding

In some cases, a trusted individual (such as a family member or representative) may manage care arrangements on your behalf.

Why complete an online care plan review?

Even though you manage your own care, completing an annual online care plan review helps us keep a record of your preferences and any changes in your circumstances. 

This information:

  • ensures your views are documented
  • can be referred to in any future assessments
  • helps us better understand the needs of residents in our community

Completing the review is optional, and you remain in full control of your care choices.

If you are unable to use the online form but would still like to keep us informed of your care needs and preferences, you can request a paper copy by emailing optreview@bedford.gov.uk

We can send the review by post or email, depending on your preference.

What if my circumstances change?

If your care needs or financial situation change, you have two main options:

Contact your care provider directly

If you arranged care through an independent agency or provider, you should contact them directly to make changes to your support package.

Request an assessment from Adult Social Care

If you would like help with planning your care and support, or if you believe you may now be eligible for financial assistance, you can request a care needs assessment from us.

You can do this by completing a self assessment.
