Physical disabilities

If you who have a physical disability and are over the age of 18 years and under 65 years of age, the Physical Disabilities team may be able to help you.

We provide support to increase your independence such as:

  • advice and information to access community services
  • reablement care to regain independence
  • equipment or assistive technology to help you in the home or to get out and about
  • a Direct Payment so that you can arrange your own care and support so that you can have choice and control over when and how you have your needs met
  • home care
  • respite
  • daytime activities
  • nursing /residential care

When you contact us to ask for help we will ask you a series of questions. The answers to these questions will help us to decide what we can do to help you.

Depending on your answers you may be eligible for an assessment under the Care Act.

Sometimes we assess your needs by telephone and sometimes in person by visiting you. It depends on your individual situation.

We also offer an assessment to anyone who is helping to look after you without payment.

All long term care and support is financially assessed. You can request a pre-financial assessment if you are concerned about costs.

We also support referrals for sheltered housing.

Your needs will be reviewed on a regular basis and the support will be adjusted according to your needs. 

We work closely with doctors, nurses, occupational therapists, physiotherapists, care agencies and other specialist services.
