We want you to be as healthy as possible, both physically and mentally.

Support from the 16+ Supporting Future Team

We will:

  • give you information on healthy living such as having a healthy diet and stopping smoking
  • support you to register with a GP and provide information on getting help to pay for prescriptions
  • support you to move from CAMHS to adults' mental health services
  • give information about services that are available locally, such as counselling, health centres and drop-in sessions and support you to attend
  • give you support and advice with transports costs when attending health appointments
  • support you to do the best for your child, if you are a parent

Your Health Passport 

If you live within Bedford Borough, the 16+ Supporting Futures Team will send you a Health Passport. Your Health Passport is a very important document, so you must take care of it and keep it in a safe place.

This will contain key health information from your childhood such as if/when you had immunisations and information about any current or past health needs.

Your Personal Advisers will work with you to make sure the Health Passport is up-to-date and to fill in any gaps. If you live outside of Bedford Borough, the 'Looked After Children’s Health Team' will send you a letter rather than a Health Passport. This will contain details of health services and some public health information.

Support for your mental and emotional health

We know that moving to the 16+ Supporting Futures Team can be a tricky time for you, there can be lots of changes and decisions to make and this can feel overwhelming. Your Social Worker and Personal Advisers will support you to reduce any anxieties you may have around leaving our care and independent living.

If you are working with CAMHS we will make sure you are supported to move over to Adults' Services. If you feel talking to a counsellor would be helpful, we will signpost you to relevant services and help you with a referral.

We will support you to understand your identity. We will help you make sense of your time in our care and answer any questions you may have.

We will support you to understand ‘who you are’ and can refer you to specialist organisations if you need more advice on issues like your sexuality, religion, gender or ethnicity.

Your Social Worker or Personal Advisers will make sure you have an opportunity to explore these areas and talk through any issues or questions you may have.

Moving from foster care to independent living can be very difficult and you can sometimes feel like you are on your own. The 16+ Supporting Futures Team will support you to try new things and meet new people. We will help you find activities in your local area and support you to attend events like the Children in Care Council to meet other young people in similar situations to you.
