Review of your care plan

Reviews are regular meetings that bring together those people who are closely concerned with your care, with an opportunity to review your care plan.

There are many ways of getting your views and voice heard:

  • Complete a consultation online or a paper copy.
  • Write to your social worker
  • Send an email to your Independent Reviewing Officer
  • Meet an advocate, who can attend the review meeting to share your views (An advocate is someone separate from your social worker to help support you at the meeting and put forward your views. Your social worker can arrange an advocate for you.)
  • Attending your meeting and presenting your views

Review of your care plan

We want you to advise us on:

  • who you would like to attend your review
  • decide where and what time you would like the review
  • if you would like to chair your review or part of it
  • what you want to discuss at your review (your agenda)
  • how we can help you achieve your goals

Review documents

We know filling in forms can be boring, but this review is about you:

  • what you like
  • what you don't like
  • how you feel about where you live
  • your education
  • those close to you

This will help everyone at your review to talk about what is important to you. It is really important we know your wishes and feelings. This is to make sure we are listening to you and making the right decisions with you about your life.

Your Independent Reviewing Officer will talk through the information with you before your review. 

Don’t forget, you need to be at your review to tell people what you want and what changes we can make for you.

Parents and carers consultations

It is really important that you make a meaningful contribution at the review so that an informed decision can be made about the short and long term needs in your child or young person's care plan.

As the parent or carer for a child/young person who is being looked after, it is essential that you contribute to the review meeting.

The meeting is chaired by an Independent Reviewing Officer, who is independent from the social worker. They are responsible for making sure we have the right care plan for your child or young person in order for them to achieve their full potential in their care.

How to support your child or young person at their review

  • Attend all reviews when invited
  • Think carefully how you can offer ongoing support to the young person
  • Complete the consultation prior to the meeting so the Independent Reviewing Officer can understand your views, feelings and the support you can offer
  • Inform the review of the achievements of the young person, their ongoing needs, wishes and feelings from your perspective

Your rights

Your Independent Reviewing Officer will inform you about what you need to know about your rights:

  • your right to seek legal advice
  • your right to have an advocate
  • your right to make a complaint
