Help for Adults
Adult Services
The role of Adult Services is to advise and support vulnerable
people age 18 years and over who are in need of social care
services and risk losing their independence. Where we cannot
provide services we can often provide advice and information on
alternative services in the community.
We provide services to:
- Older people
- People with housing related support needs
- People with learning disabilities
- People with mental health problems
- People with disabilities, sensory needs and long term
conditions
- People with substance misuse issues
- Carers – anybody who provides unpaid care to people in the
above groups.
We aim to support people to live independently in your own home
or where this is not possible, to provide alternative care
arrangements that allow you to be as independent as possible.
Assessment of need
When you contact us
to ask for help we will ask about your health, daily living
skills, abilities and personal circumstances. The social
care worker will gather this information with you and your
carer and agree with you what your needs are and how these might be
met. This is known as an assessment. The assessment and
any follow-up enquires by the social care worker are a free
service.
Contact details for the adult social care teams are
available here.
You can make an online general
enquiry about adult social care services. We aim to
respond to your query within 2 working days.
On line assessment services
Self assessment
gives you the opportunity to assess your own care needs or enable
someone to support you in doing the assessment. This service is
aimed at adults (aged 18 or over) who are finding looking after
themselves increasingly difficult. For example if you or someone
you know may be having difficulty getting in and out of the bath,
managing stairs or with some aspects of personal care.
Self assessment gives you the opportunity to
carry out an assessment of your needs from the comfort of your own
home as we believe that you and the people closest to you can best
assess your care needs.
